For livestock movements the owner/person in charge of the animals declares that the applicant or a competent person has inspected the herd / flock from which the animals will be consigned within the last 24 hours and believe all animals are in good health and free of signs of illness/ symptoms of disease

Attach any relevant documents in support of the application e.g. NVD, Animal Health Declaration, Veterinary Certificate
